Please note that acceptance of a grant carries with it the obligation to:

1. Provide regular status reports & updates as the program progresses;
2. Complete a program evaluation at the conclusion of the project (sample attached)
3. Keep accurate records, especially records of costs and expenses;
4. Provide photographs of significant events that take place during the course of the program;
5. Include the following statement in any information about the program: “This program was made possible, in part, by a grant from the North Haven Education Foundation, Inc.”
6. To ensure receipt by the foundation board, please send a copy of your application to the contact information listed above, either electronically or U.S. Mail.
